Absent and Bus Notes

Absent Notes:

Please send all absent notes to the front office or email Sarah Campbell at sarah.campbell@sumnerschools.org

Attendance Policy:

  • Verified Absences - Doctor notes will verify student absences for the dates listed on the note itself. Always be sure to request an absence note every time you visit the doctor/dentist/hospital/court. These absences are considered verified absences.
  • Excused Absences - Parent notes can excuse absences when a doctor's visit is not necessary. Only 5 parent notes are accepted each semester. These absences are considered excused absences.
  • Once a student accumulates 5 unexcused absences, parents are required to meet with a school administrator to sign an attendance contract. For this reason, it is important to keep all student absences verified or excused.

Bus Notes Requests Rules:

  • All bus requests must be turned into the front office or emailed NO LATER THAN 12:00pm the day of the bus ride.
    • If emailing, email Joan Petre at joan.petre@sumnerschools.org
    • Call the front office at 615-824-3456 to let HMS know a bus note is coming thru email.

Required information for a bus note:

  1. Your student's First and Last name.
  2. Dates your student will need to ride the other bus.
  3. The name of the student your student will be riding home with on the bus; OR
  4. If they are not riding home with another student, then we will need the address of where they are riding to AND the name of the person that lives at that address.

Guidelines:

  • Only students that live in Hawkins' zone can ride our buses.
  • Students enrolled as out-of-zone are not allowed bus notes.
  • Long-term bus note requests can be completed for students who will need to ride another bus multiple times during the school year. (Such as a student riding to another family member, a friend's house for afterschool care, etc.)
  • Buses cannot be used as transportation for a sleepover or party where multiple students would be going home with the same student. Buses have limited seating and additional students going on one bus could cause overcrowding.
  • Students will need a bus request for every time they will be riding another bus unless they have a long-term bus note completed in advance.
